Recent Clinical Evidence

In recent years, multiple randomized controlled trials (RCT’s) suggest that Botox®  injections significantly improve depressive symptoms compared to placebo.

Here are the most recent ones:

Time Magazine botox

Key Findings:

  • Wollmer et al. 2021 -Found that patients treated with Botox® showed a significantly lower incidence of anxiety than controls. The effect appeared not only in glabellar (frown line) injections but also in other injection sites*.
  • Crowley et al. 2022 – A meta-analysis of five randomized controlled trials confirms the antidepressant efficacy of Botox® injection.
  • Li et al. 2022 – A randomized controlled trial showing significant improvement in depression severity vs. placebo following Botox® injection.
  • Shu et al. 2024 – A randomized controlled trial found that both 50U and 100U doses of Botox® were equally effective in reducing symptoms of mild to moderate depression and anxiety.
  • Demchenko et al. 2024 – A comprehensive review of relevant clinical trials concluded that Botox®  is safe, well-tolerated, and effective for major depressive disorder, with mild and temporary side effects. Additionally they found it to be effective for social anxiety.

Across studies, side effects are minimal, typically limited to mild injection-site tenderness or temporary asymmetry.

* This study shows that the mental health benefit is not specific to frown-line injection sites and cannot be explained solely by the aesthetic effect of softening those lines.

Frequently Asked Questions

At this moment, insurance does not cover Botox® or Xeomin® injections.

Both Botox® and Xeomin® contain the same active ingredient — botulinum toxin type A (BoNTA) — and work similarly to relax muscles and reduce wrinkles. However, there are key differences in their formulation and properties.

Key Differences:

Formulation:

  • Botox®: Contains accessory proteins (complexing proteins) surrounding the active toxin
  • Xeomin®: “Naked” neurotoxin with no accessory proteins, making it a purer formulation

Antibody Resistance:

  • Botox®: Accessory proteins may increase the risk of developing antibodies over time, potentially reducing effectiveness
  • Xeomin®: Lower risk of antibody formation due to purified formula, making it ideal for long-term use

Clinical Effectiveness: Both products have comparable efficacy for treating depression and cosmetic concerns. Clinical studies show similar antidepressant effects and duration of action (24+ weeks for mood benefits). The choice between Botox® and Xeomin® often comes down to individual patient response, provider preference, and history of antibody development.

At Kadima, we offer both Xeomin® and Botox® injections.

For depression treatment, Botox® is injected into the glabellar region — the area between your eyebrows where frown lines appear. This treatment specifically targets the corrugator and procerus muscles, sometimes called the “grief muscles,” which control frowning and express negative emotions.

Botox® injections for depression are generally well-tolerated with mild and temporary side effects. Most adverse effects are related to the injection site and resolve on their own within a few days.

Common Side Effects:

  • Injection site reactions: Bruising, redness, swelling, or tenderness
  • Headache: Typically mild and short-lived
  • Temporary muscle weakness: In the treated area
  • Drooping eyelid (ptosis): Rare, but can occur if the product migrates

Serious Side Effects (Rare):

  • Difficulty swallowing or breathing
  • Muscle weakness beyond the injection site
  • Allergic reactions (rash, itching, difficulty breathing)

Clinical studies confirm that Botox® for depression has a strong safety profile with no significant long-term adverse effects reported. Most patients experience minimal to no side effects, and the treatment does not require downtime.
